Here is a survey where you can share your experience training with your guide dog. Your participation is completely confidential and voluntary. The results will be compiled periodically and will be publicly viewable.

If you have attended more than one program please feel free to complete the survey once for each program you have attended. If you have received more than one dog from the same program, please take the survey only once based on your experience with your most recent guide dog.
